| Community Funds Grant Information |

| Are You Eligible for a Community Fund Grant? Check the following: |
- Applications are considered for projects or programs that provide a direct benefit to the people of a respective community as defined by the school district boundary.
- The applicant should be a 501(c)3 charitable organization. If you are not a 501(c)3 but believe your program has a charitable purpose, contact Kristen Duesdieker, grants and scholarships manager, (608) 758-0883 or 1-800-995-2379, or kristen@cfsw.org. Individuals are not eligible for community fund grants.
- For details on priority areas of funding and potential grant award amounts, contact the respective donor services representative listed below.

| How to Submit an Application: |
- Click on the name of the community fund below to access the application form.
- Complete the form online and then print it out. You may save the digital copy to work on in stages.
- After printing the completed application, obtain the required signature(s) and additional documentation per the Proposal Outline at the front of the form.
- Submit the original and a copy plus documentation to the mailing address affiliated with your specific community fund by the deadline date.
- ALL APPLICATIONS MUST BE RECEIVED IN THE OFFICE LISTED BELOW BY
4:00 PM ON THE DEADLINE DATE. If the deadline falls on a weekend or holiday, the deadline will be the next business day.
